California Healthy Kids Survey 2011-2012

“Hayward Unified School District will be participating in taking the California Healthy Kids Survey (CHKS). The CHKS provides our District and Schools with information regarding health and safety from the students perspective. The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of the school..

 

Students in 7th, 9th and 11th grades will be taking the survey during the months of February and March. If you wish to view the survey,  please click on the attached links. Participation is encouraged but if you do not want your child to participate in taking the survey please respond in writing to your schools by February 10, 2012.”
